CTR Form Download
Directions for completing CTR Report Form:
* This form is an editable PDF form. If you do not have Adobe Acrobat Reader on your machine, either installed as a separate program or plug-in to your current browser, you will NOT be able to edit and save this form. You can obtain a free copy of Adobe Acrobat HERE (link to acrobat reader download). Depending on your specific system type (i.e. Apple, Microsoft, Linux) and browser software (i.e. Internet Explorer, Safari, Firefox) you will be presented with several options of entering and saving your CTR report form for submission to Triangle Transit. Also dependent on your specific system and browsing software, the form's Submit buttons may be disabled. In this event, simply right-click and save the form to a specific location on your machine.
**There is a possibility upon opening the PDF file, either in a browser window or directly through Adobe Acrobat, where a "This Operation is not permitted" warning to appear. Simply click the "OK" buttons until the several warnings disappear and continue to edit the form.
IN BROWSER WINDOW:
1. If an Acrobat Reader plug-in is installed on your machine, the PDF will load into a new browser window like a typical web page.
2. You can then edit the file as if you were completing any other online form. WARNING: DO NOT CLICK THE BACK BUTTON OR CLOSE THE WINDOW - YOUR INFORMATION MAY BE LOST
3. Upon completion of entering in your information, you may use either SUBMIT FORM buttons located either at the top right-hand corner of the form or on the last page (p.13) of the form. Pressing one of these buttons will bring up a screen where the program will ask you to indicate an option to send your form.
Option 1: Desktop Email Application if you wish to submit your form using Microsoft Outlook, Apple Mail, Outlook Express, or any other email client. Choosing this option will save your information as a readable file and place it in a new email to be sent immediately.
Option 2: (BEST OPTION) Internet Email if you choose to use an internet-based email service like Gmail, Yahoo, or Hotmail. Selecting this option will give you the option to save the PDF form file in a location you specify on your machine. This file can then be attached normally as an email attachment and sent using any method of your choosing. Using this option, you will have to manually enter the return email address: .(JavaScript must be enabled to view this email address)
DIRECT DOWNLOAD:
1. Depending on the system, your machine may download the PDF file directly to a default folder on your machine or desktop. You may also directly download this form by right-clicking the link and "Save As" or "Save linked file" then specifying a location.
2. Once download is complete, simply open your CTR form from the location it is saved and follow the same instructions from step 2 above.
3. Using this option, you will have to manually enter the return email address: .(JavaScript must be enabled to view this email address)
**This form file does NOT communicate directly the Triangle Transit website.
